Any eligible Hoosier can self-register for a vaccine appointment. A friend or family member also can make an appointment on behalf of a qualifying individual. But the person registered will be required to provide photo identification before they will be given the COVID-19 immunization.

Walk-Ins will be accepted under the conditions below and appointments are still accepted. Walk-ins will be accepted under the conditions below and appointments are still accepted. Persons age 5 thru 17 must be accompanied by a parent or legal guardian.
Regardless of where in Indiana you received your first dose, you can receive your second dose under these conditions:
If your first dose was Pfizer, at least 21 days must have passed since your first dose.
If your first dose was Moderna, at least 28 days must have passed since your first dose
You MUST present the vaccination card you were given when you received your first dose.
